We have an immediate opening for a reliable LPN (Licensed Practical Nurses) for a 13 week contract for 12hr night shifts at a Skilled Nursing Facility in Northern Arizona.

Location: Winslow, AZ
Facility type: Skilled Nursing Facility
Pay rate: $1,620 per week
Start Date: 12/7/2024
Type: 13 week contract
Shift/Hours: 12hr shifts (Day & Night)


Duties Include:
  • Assesses patients by physical examination, including pertinent diagnostic testing to determine health status.
  • Administers medications and treatments.
  • Participates in the care planning process.
  • Communicates with physicians regarding changes in resident’s conditions, diagnostic test results, etc.
  • Documents assessments and care in compliance with standards of care and company policy.
  • Educates patients and their families on health-related issues.
  • Completes required forms and documents in accordance with company policy and state and/or federal regulations.

Requirements:
  • Current and valid LPN license
  • Minimum 1-2 years of experience
  • Current BLS certification is required
  • Bachelor’s degree from an accredited school of nursing is required
