Practice Highlights / Qualifications
Full-time post-acute care position
Round in skilled nursing facilities
Perform patient assessments, physical exams, and care planning
Collaborate with physicians, nursing staff, and interdisciplinary teams
High level of clinical autonomy with strong support structure
New York NP or PA license and DEA required
Experience in post-acute, acute care, ED, or clinic settings preferred
