Expected income range $103,000–$122,000 annually
No cap on productivity bonus income potential

Practice Highlights / Qualifications
Full-time position (5 days per week)
Round in skilled nursing and post-acute care facilities
Perform patient assessments, physical exams, and ongoing care management
Develop and manage individualized treatment plans
Collaborate with physicians, nursing staff, and interdisciplinary care teams
High level of autonomy with strong clinical support
North Carolina NP or PA license required (or ability to obtain)
DEA required
Experience in post-acute, acute care, emergency medicine, or clinic settings preferred
