Practice Highlights / Qualifications
Outpatient primary care setting

Collaborative practice under a supervising physician
Interdisciplinary team support and technology-enabled care model
EHR: Elation
Master of Science in Nursing from an accredited program required
ANCC or AANP certification required
Texas Nurse Practitioner license required or ability to obtain
Family, Adult Gerontology, or Primary Care certification preferred
Must meet employer credentialing standards
Senior care, gerontology, post-acute, or value-based care experience preferred
Spanish language skills preferred
