Nurse Practitioner (NP) / Physician Assistant (PA) – Post-Acute Care – Hartford, CT
Seeking a Nurse Practitioner (NP) or Physician Assistant (PA) for a full-time Post-Acute Care opportunity in the Hartford, Connecticut area. This role offers a flexible weekday schedule, strong clinical autonomy, comprehensive onboarding, and uncapped earning potential while caring for patients across skilled nursing facilities.

Expected compensation range: $158,000–$188,000 annually with no cap on productivity income potential.

Practice Highlights / Qualifications
Full-time post-acute care position
Round in skilled nursing and long-term care facilities
Conduct patient assessments, develop treatment plans, and manage ongoing care
Collaborate with physicians, nursing staff, and interdisciplinary care teams
High level of autonomy with strong operational support
Connecticut NP or PA license required (or ability to obtain)
DEA required
Experience in post-acute, primary care, hospital medicine, emergency medicine, or geriatrics preferred
