We are looking for physicians who want to own their own Direct Primary Care practice!Â
This is not an employed position. The physician forms and owns 100% of a Texas PLLC, makes every clinical decision, and contracts with the MSO for the brand and non-clinical support.

• Model: Cash-pay membership primary care for adults and children; no insurance billing
• Support available: Brand, operations playbook, technology, lab and supply pricing, clinic space, equipment leasing, and optional access to working capital through Merlin entities
How the Physician Pays the MSO
The physician’s practice pays the MSO directly. Every amount is fixed in writing at signing and never depends on the clinic’s revenue, profit, or patient count.
Item Terms
Annual management fee paid monthly. Covers the brand license, operations playbook, technology, vendor pricing, and ongoing support.
First-year deferral The Year 1 fee can be deferred and repaid on a fixed schedule after launch. The MSOÂ sets out the schedule in the deal terms.
Clinic space Leased from a MSOÂ entity at fair market value rent, with an option to buy later.
Medical equipment Leased from MSOÂ at fair market value, with an option to buy later.
Working capital Available as a loan at a stated interest rate, repaid on a fixed schedule, subject to approval.
Ideal Candidates
Must have
• Board certified or board eligible in Family Medicine, Internal Medicine, or Med-Peds (Internal Medicine owners see adults only)
• Active, unrestricted Texas medical license (or license in process) and DEA registration
• Clean Texas Medical Board and malpractice history
• Wants to own and build a practice, not only to see patients
• Comfortable with a direct-pay membership model and a smaller panel
• Able to contribute some personal capital or sign a personal guarantee on launch financing
• No restrictive covenant that would keep them from practicing in the target market within the next 12 months
Strong plus
• Roots and a professional reputation in the target community
• Leadership or business experience (ran a clinic, managed staff, owned a business)
• Bilingual English and Spanish
Not a fit
• Wants a guaranteed salary or an employed role
• Wants high-volume, insurance-based practice
• Is looking for a passive investment rather than running the clinic
